WEST PERTH FOOTBALL CLUB MEMBER’S SURVEY

DEAR all West Perth Football Club Members, you are invited to participate in this project, which is being conducted as part of the requirements of a unit at Edith Cowan University.
 
The purpose of the project is:
To gain an understanding from members of their current attitudes and customer satisfaction levels towards their membership, club merchandise and match day experience. This data will be used to improve the elements of the club covered in the survey. Suggestions for improvements, made by members, will be considered for future seasons.

Because you are currently a member of the West Perth FC. Potential participants will be selected from the WPFC membership database. 
 
If you choose to participate in this project you will be asked to:
Complete an anonymous questionnaire which will take approximately two minutes to complete
 
As noted above, this is an anonymous questionnaire. Please ensure that you do not write your name, or any other comments that will make you identifiable, on the attached questionnaire. 
 
The information will be used to complete the requirements for the unit noted above, and only the student(s) and the unit coordinator will have access to the information. At the completion of this unit, all data will be destroyed.   
 
Participation in this project is voluntary. If you choose to participate, you are free to withdraw from further participation at any time without giving a reason and with no negative consequences. There are no risks associated with this research to participants. By completing the questionnaire you are consenting to take part in this research. As such you should read this information letter carefully as it explains fully the intention of this project.
